Bellefontaine Habilitation Center is no longer accepting referrals
for admission. All information regarding services provided by the Department
of Mental Health is initiated by calling, writing or going to the Regional
Center that serves your county. Services for the St. Louis area may be
obtained by contacting the Eastern District Regional Centers listed below:
According to Missouri Statutes, individuals admitted to a habilitation center or community residence must have a developmental disability that occurred before the age of 22 (which is the developmental period) and that it is likely to continue indefinitely. The Regional Centers serve as the entry and exit points to the Department of Mental Health services. They perform intake and assessment activities that help to determine if an individual is eligible for services. After a person is determined to be eligible, the Regional Center Service Coordinators work in partnership with the individual and family to identify appropriate services and supports. Placement may be made at one of the habilitation centers. The coordination of services within BHC is guided by an individualized development plan called a Person Centered Plan (PCP). The PCP guides the supports, skill development and community activities of each person. The plan is regularly reviewed and updated by a team of trained professionals, the individual and the family and legal guardian. |
